Passports
The Library is a Passport Acceptance Facility. This means we can accept applications for NEW adult passports and NEW or RENEWAL minor passports. How do you know if you need to come to an acceptance facility?
HERE IS WHAT YOU NEED TO KNOWSee travel.state.gov for full requirements
You MUST apply in person at an acceptance facility if any of the following are true:
1. You have never been issued a U.S. passport or
2. You were under age 16 when your previous passport was issued or
3. Your most recent U.S. passport was issued more than 15 years ago or
4. Your most recent US passport was lost/stolen or
5. Your name has changed since your previous U.S. passport was issued and you are unable to legally document your name change.
BEFORE your appointment:
it is important to carefully check travel.state.gov and their directions on Form DS-11 to avoid delays in processing.

Q. How do I make an appointment?
Appointments can be made through our calendar > type "Passport" in Keyword > select an appointment > click the "REGISTER" button and complete the passport appointment form. If you see "FULL", that means the slot is booked.
We add new appointments as we are able. Let us know if you cannot make any of the available dates, and we will do our best to accommodate you.
One person or family up to 4 members per appointment (contact us if more than 4 per family).
We will send you an email confirming your appointment, along with details as to what you will need to bring with you.

Q. What do I need to bring with me?
You will need to bring proof of ID, such as a driver’s license; proof of U.S. citizenship, such as an original or certified birth or naturalization certificate, or a previous U.S. passport; and the appropriate fees.

Q. Can I have my passport photo taken at the Library?
Yes, we take passport photos, for new OR renewal passports (but we cannot accept renewal applications; you must mail those on your own). The charge is $10.00.

Email it to us and we'll print it for your appointment. Be sure to carefully check the photo regulations on travel.state.gov .
Q. What are the fees?
TO THE LIBRARY*:
1. Execution Fee (required): $35/per new application
2. Photo Fee (optional): $10 to print a photo
* Fees to Butterfield Library can be made by cash, money orders, personal checks, or credit cards.
TO THE US DEPARTMENT OF STATE*: Fees
*Fees to the US Department of State MUST be a check or money order (no starter checks).
Q. I have a Minor, what do I need to know?
Age 15 or under: Both parents (or the child’s legal guardians) must appear with the child. Click here for more information if both parents cannot be present. Evidence of the child’s U.S. citizenship, such as an original birth certificate; Evidence of the child’s relationship to parents (original birth certificate); and Parents’ government-issued ID (such as driver’s license)

Q. Do you do renewals?
***WE CANNOT DO RENEWALS *** RENEWALS ARE COMPLETED and MAILED ON YOUR OWN . We can offer help if you have questions, but cannot accept or mail your application.
HOW TO APPLY for a PASSPORT
Follow the instructions below exactly.
STEP 1:
Complete and Print Form
√ Enter the information online and then print Form DS-11
or
√ Print a blank Form DS-11 and complete by hand in black ink
√ Do not sign the DS-11 form until the Passport Agent instructs you to do so.
STEP 2:
Submit your completed application form and the following additional documents:
Proof of U.S. Citizenship (one of the following):
√ U.S. birth certificate or certified copy
√ Previous U.S. passport (can be fully-valid or expired, but must be undamaged)
√ Certificate of Naturalization
√ Certificate of Citizenship
√ Certificate of a U.S. Birth Abroad
Proof of Identity (one of the following; originals only):*
√ Valid in-state driver's license (not temporary or learner's permit)
√ Valid out-of-state driver's license (not temporary or learner's permit) If you have an out-of-state license, you
MUST provide an additional proof of identity
√ Previous U.S. passport book or passport card (can be fully valid or expired, but must be undamaged)
√ Certificate of Naturalization with identifiable photo attached
√ Certificate of Citizenship with identifiable photo attached
√ Government employee identification card (federal, state, county, municipal)
√ Official U.S. military or military dependent identification card
√ Current (valid) foreign passport
Photocopies:
Copy (front and back) of both citizenship and identity evidence. REQUIRED.
STEP 3:
Pay the Applicable Fees
TWO payments are required:
1. One made payable to the U.S. Department of State for the cost of your passport.
Payment to the U.S. Department of State must be in the form of personal checks or money orders (no starter checks accepted).
2. One made payable to Butterfield Library for the application fee and/or photo. Payment to the Library can be check, money order, or credit card.
Non-Refundable Fees: The passport application fee and the execution fee are non-refundable by law.
